I have been in healthcare for the past 18 years as a Registered Nurse, first working for the first 9 years in Emergency Medicine, and after some time as an RN, I decided to expand my knowledge and career by pursuing an advanced degree. In 2021 I graduated with my Master’s Degree from Samuel Merritt University as a Family Nurse Practitioner (MSN, FNP-C). For the last 9 years, I have served my community as a Home Health RN, and in that time, I have visited patients in their private homes, and in Assisted Living Communities. I highly value this personal approach to healthcare, and now I hope to continue this wonderful work as your Provider to complete your 602a forms in order to assist you in making this move into your new Assisted Living Community! Moving can be a highly stressful time, but my goal is to help make at least this small part of your moving process an easy and simple one, by being there for you in completing this form for you seamlessly and quickly, so that you can focus your attention on the other aspects of the moving process. I will complete your form and TB test and then be your liaison with your new community in delivering the form directly to them, to allow you to focus on other moving tasks.
